Overview

A bespoke provision of a single point of access for ophthalmology referrals from primary care (from optometry and GP practices) with referral triage undertaken by experienced optometrists to ensure referral quality and accuracy.

Redirect patients to appropriate care settings, freeing up capacity in ophthalmology and ensuring timely care delivery in primary care settings  closer to patients’ homes.

This includes administrative hub that provide  support for patients and practices for referral related queries.

Service Information

Single Point of Access 

  • Provides a single destination for ophthalmology referrals (non-emergency)  to simplify the pathway for optometrists and to remove administrative load  from GP practices and hospitals
  • Tailored approach taken to meet the local system’s needs, with deflection  to primary care pathways and clinic allocations to improve efficiencies in  ophthalmology departments
  • Provides and administrates patient choice
  • Data used to inform future commissioning priorities

Triage Service 

  • Carried out by experienced optometrists with a robust quality assurance  programme and live audit
  • Ensures optimal utilisation of local community pathways with accurate  allocation to sub specialty ophthalmology clinics, adhering to local  guidelines
  • Thematic analysis of referrals to inform local education
  • Communicates with primary care to improve referral quality at source
