There are already a large number of patients requiring glaucoma care and this is projected to grow still further in the next 10 years.

Extensive  resources in primary care estates, equipment, and clinicians represent a significant opportunity to deliver high  volumes of care close to home.

Primary Eyecare Services have developed a flexible glaucoma monitoring offer with a range of options to suit the requirements of different patient groups and different systems, which align  to recommendations from GIRFT, CCEHC, and NICE.

Accreditations
Accreditations vary depending on the monitoring service being delivered and local accreditation requirements.  Email hello@referral.support with your area and the service you wish to deliver and the team will guide you on  this.

Three Models and a Range of Options

  1. Virtual clinics provided by optical practices in community, with review by Trust clinicians.
  2. Discharge of low-risk cases into the care of primary care optical practices with a management plan.
  3. Discharge to our Consultant-led monitoring service. Patients are examined in optical practices and virtually  reviewed by our own glaucoma specialist clinicians.

These models can be combined to suit different local resources and patient cohorts. For example, low risk  patients can be discharged to local practices with a three-year management plan, after which they are virtually  reviewed by Trust clinicians. Within the same system, moderate risk patients may be examined in community  but have their notes virtually reviewed after every episodes.

IT, Failsafe and Governance
We use an exclusive, secure, web-based IT platform to transfer clinical data between hospital sites, optical  practices and virtual review facilities. This includes clinical summaries, imaging (fundus photo or OCT) and  visual fields.

Our Failsafe team track FTE/DNA and ensure timely review, ensuring strong service oversight and patient  safety.

Our experienced clinical lead optometrists work with our glaucoma specialist clinicians to maintain high  standards of data collection, robust decision-making processes and clinical audit.

Patient Focused 
The local optical practices we work with are familiar primary care settings with excellent patient access. We  collect patient-reported outcomes, with 98% of patients stating they would recommend the service. Where  Primary Eyecare Services deliver virtual review and clinical decision-making, our glaucoma team can be contacted by telephone or email.
